Program Coordinator (Seniors Community Connector)

STATUS: Temporary Full Time

HOURS OF WORK: 35 hours per week
SALARY: $36.09 to $42.45 per hour + comprehensive benefits package

The Program Coordinator (Seniors Community Connector) plays an integral role in bridging the gap between healthcare and social care. They will partner with community organizations to provide non-medical support to improve the overall health and well-being of older adults in the community. This position will work with older adults who are referred from Health Care Professionals and community partners to connect them with community supports and services through referrals, applications, advocacy, and introductions. This role includes a diverse range of responsibilities - from co-developing personalized wellness plans, to providing practical support to older adults and their families, to establishing and maintaining relationships with people and organizations in the community offering supports to older adults. You will work in collaboration with health care referrers, other external Seniors Community Connectors, community organizations and staff teams to create an environment where seniors at-risk of frailty will be able to access resources and support. The goal of this position is to assist older adults to age safely by providing them with the social connections they require and to build and maintain partnerships and connections of community based seniors services.
 
If you have the following knowledge, skills and attributes, we want to meet you!
  • Certificate, diploma or degree in social service or related human services field or a combination of relevant education and experience deemed suitable by the employer
  • Strong knowledge of seniors' issues and challenges related to healthy aging
  • Experience working with seniors and diverse populations from different cultures and socio-economic backgrounds
  • Knowledge of community resources, programs and services
  • Minimum two years recent related experience
  • Standard First Aid and CPR Level "C/AED"
  • Driver's License for the Province of British Columbia
  • The successful applicant must be able to pass and maintain a clear Police Information Check including Vulnerable Sector Check
  • Must be available to work a flexible schedule and a variety of shifts based on operational requirements.
 
Job Skills and Abilities:
  • Demonstrated ability to connect with and support seniors
  • Sound knowledge of community resources related to seniors such as government agencies, not-for-profit organizations and other community groups
  • Ability to work with seniors in developing and implementing or connecting seniors to a wide range of opportunities, programs and events that reflect their interests and needs.
  • Experience with developing wellness plans, referrals, and preparing reports
  • Demonstrated skills in the areas of crisis intervention and conflict resolution
  • Demonstrated ability to recruit and supervise volunteers
  • Strong collaborative skills and proven ability to establish and maintain effective working relationships with all internal and external contacts
  • Excellent written and oral communication skills and ability to clearly explain instructions to others
  • Ability to work independently with strong time management and organizational skills
  • Strong computer skills
  • Knowledge of equity, diversity and inclusion principles
